When breast implants are placed the overlying breast tissue, especially over the lower breasts, becomes thin. This occurs less in the upper breasts because of the presence of the pectoralis muscles. Unfortunately, when the implants and capsules are removed the thin breast tissue of the lower breasts contracts but the volume is never replaced. This can result in constricture or tightening of the lower breasts, creases and downward pointing nipples. To correct this would require fat grafting.
