This case illustrates a couple of things. First, this patient's skin color is known as a Fitzpatrick 4. This is typical of Hispanics, Asians and some Middle Easterners. I don't like to use the periareolar incision for these patients as it could become hypopigmented (less color, as in this patient) or hypertrophic (thickened). She insisted on the incision. If the color does not improve I will suggest she have the scar tattooed. Second, her areolas have very indistinct borders and thus again the periareolar incision would not be the best choice. Finally, both of her breasts have constrictures at the bases that were released improving the overall appearance.
